OSS implements student organizations activity green form

Starting academic year 2019-2020, student organizations holding in-campus and off-campus activities will now be required to submit a green form, which certifies compliance with parent consent form (PCF), as stated by Office of Student Services (OSS) Dean Arch. Antonio Gutierrez Jr.

The green form will act as validation that the student organization submitted PCFs, after accomplishing event proposals to the OSS, and as a gate pass to monitor the number of participants, including the accompanying faculty member.

On what necessitated the implementation of the green form, Gutierrez said “security at safety ng estudyante,” emphasizing the importance of the form for off-campus activities: “Kasi may mga nakakalabas na off-campus [activities] na minsan wala palang [accompanying] faculty.”

Relatively, there have also been previous cases where students participate in activities without submitting the required parent consent form (PCF).

In addition to student security and safety, the green form also proves the activity to be “genuine.”

“‘Di ba may mga certain activities [na] ginaganap tapos hindi natin alam pag may nagtanong, halimbawa security, malalaman ng security, ng academic community, kung mag-inquire sila na iyan ay genuine activity, ipapakita [lang] nila yung green form.”

Serving as a student organization event report, the green form will also be an additional attachment to the student organization activity accomplishment report.
