No student entry yet; health checklist forms strictly required
As per the General Services Office (GSO) revised safety and health protocols, no student entry is allowed but their parents or guardians can be permitted for in-campus office transactions, along with school faculty and staff for University work after emailing GSO their health checklist forms.
In compliance with existing government declarations and issuances on the coronavirus disease 2019 (COVID-19) pandemic, parents or guardians are advised to present a government-issued ID and their child’s ID upon entry.
GSO Director Nathaniel Golla shared that the protocols are still subject to change “depending on prevailing situations and government policies”.
Health checklist form
The protocols mentioned that school faculty and staff are required to pass a health checklist form to GSO via email a day before their campus entry.
Meanwhile, expected guests “shall be governed by the Permit to Enter policy with the submission of Health Checklist Form” at least three days before their scheduled visit. Moreover, an endorsement from the Vice-Chancellor or President is required, limiting their movement to places they are expected in.
As for walk-in guests, they will only be allowed in for official business, subjecting them to “strict screening and health protocols”.
GSO will also issue the list of third-party providers and their employees that are allowed to enter the campus. The safety officer of the company is required to submit a report to the Campus Safety Office (CSO) on the stay-in third party employees’ health status every 14 days.
Additionally, DLSU-D’s official twitter posted that QR codes can now be used to automatically check-in on the date of your campus visit.
As of press time, wearing of face shields and face masks inside the campus are required to comply with the Department of Trade and Industry (DTI) and Department of Labor and Employment (DOLE) joint memorandum, effective immediately as of the return of general community quarantine (GCQ) this August 19.
